This case deals with the management of a young woman who attends the Emergency Department with a short history of severe lower abdominal pain. The scenario of acute lower abdominal pain in a young woman is a common one, frequently seen in general practitioners' rooms and emergency departments.  Apart from the challenge of arriving at a prompt and appropriate diagnosis, the place of various investigations must be put in perspective. This case discusses some of these issues.
